The 22-year-old kept a clean sheet in his side's 2-0 victory over Austria Klagenfurt to clinch the title by two points ahead of Red Bull Salzburg.
That win completed a domestic double for Graz after they beat Rapid Vienna to win the Austrian cup at the start of the month.
Jaros completed his temporary switch to Austria in January and went on to make 21 appearances in all competitions.
The Czechia youth international joined Liverpool from Slavia Prague in 2017 and penned a new deal with the club before his loan to Graz.
