Jota was introduced off the bench in the 83rd minute of the last-four showdown in Munich, which saw Portugal come from behind to win 2-1.
They will now meet Spain or Ibrahima Konate's France on Sunday in the showpiece at Allianz Arena.
The other semi-final takes place on Thursday evening.
