The Liverpool head coach guided his side to glory in the division at the first time of asking, with the Reds winning the 2024-25 title with four games to spare.
At the time of writing, Slot’s men are 15 points clear at the top of the table and have lost only three times in 36 matches during his debut campaign at the helm.
The Dutchman is up against Nuno Espirito Santo, Thomas Frank, Eddie Howe and Vitor Pereira for the prize.
Voting is open here until 12pm BST on Monday (May 19). The winner will be announced on May 27.
