Phil Dowd will referee the FA Cup final between Liverpool and Chelsea on May 5 at Wembley.
The 49-year-old will be assisted by Stuart Burt (Northamptonshire FA) and Andrew Garratt (Staffordshire FA) along with fourth official Michael Jones (Cheshire FA) and reserve assistant referee Simon Long (Cornwall FA).
Staffordshire-born Dowd was the fourth official for the 2006 FA Cup final at the Millennium Stadium when Steven Gerrard's heroics secured a memorable seventh FA Cup victory for the Reds.
He also refereed the 2010 League Cup final and 2011 FA Community Shield.
"I was honoured and privileged when I found out (about this year's final)," Dowd told The FA.com. "You obviously hope as a referee that you might one day be considered for the FA Cup Final so it will certainly be the highlight of my career so far."
